Global Referral Form Instructions

This is a referral form that is issued upon completion of your pool class. It is only required if you elect not to do your dives inside Scuba Network. The cost for a Global Referral includes your registration costs as well as your physical c-card upon your return.
Any instructor from any agency can evaluate your skills, and then you bring the form back to us and we certify you with SDI. We can accept a pdf scan of this form back, but not a picture file. Please feel free to use our Global Referral Email Template to send to dive shops ahead of time to make your process smoother. Just copy and paste it, and input your details of travel.
The pre-requisite to obtain this form is completion of SDI Open Water Scuba Diver Academics, and our Confined Water Pool Class.
